Parents – Transition Tools


There are several tools that may be helpful as you help your son/daughter prepare transitioning to the post-secondary setting/life after high school:

First, the Toolbox for Self-Determination from I’m Determined website is an EXCELLENT resource to help you develop your son/daughter’s self-advocacy skills starting with everyday examples.

Second, this  Parents – Transition Skills Checklist is a good reference to help families think about a variety of skills their child will need for independence.

The Journey to Life After High School: Road Map for Parents of Children with Special Needs  is a useful handbook to help navigate the multitude of options that families need to consider as their child is getting ready to transition to life after high school.

Last, this Junior-Senior Checklist may be a simple guide to help remember the basics that need to be taken care of as your child gets prepared for the post-secondary setting.
