Transition Planning Rubric
The Transition Planning Rubric can be used to note where students’ skills are in the areas of living, learning, and working. It can be done in conjunction with the parents, student, and teacher to compare perspectives, or only by the student and teacher, or simply by the teacher. This rubric may also be used in sections, if only specific areas need to be assessed.
This tool is a great visual, and specific sections could be used jointly if the teacher, parents, and student each rate where they see the student’s current skill levels.
Several teachers are using this annually with their students, updating the progress that has been made in each area.
