Waiver Application Options
If the family feels that they may benefit from waiver services to provide additional supports (both now as well as in the future) for their child, there are several options to consider:
First, if the child is currently on Medicaid, then the process is much easier. The parent(s) need to complete the one page linked form: DHS – Home & Community Based Waiver Application (for current Medicaid recipients only). Please know that you can check multiple waivers on one application, and it is recommended to do so. The family can always change to a different waiver once they come off the waitlist IF that next waiver provides additional services and supports needed.
If the child is currently NOT on Medicaid, then the family needs to complete either an online application or print and complete the paper version of the application. (We STRONGLY suggest completing the paper version!)
